Wigan Borough Clinical Commissioning Group (WBCCG) became a statutory body in April 2013. The CCG works across three townships (Ashton, Leigh and Wigan) within its 6 localities. The CCG serves a population of 320,000. Delivering strong clinical leadership is at the heart of the CCG’s ethos. The CCG will operate as a partnership of clinicians and managers focused on improving clinical outcomes and services for patients. The vehicle for delivering that ambition will be our localities who will act as the driver for the development of the CCGs strategies in the future.
The Wigan Integrated Care System (ICS), incorporating local health providers and the local authority. Is building a strong collaborative partnership which will be able to take shared accountability and responsibility to meet the health and care needs of the Wigan locality population. Our aim is to have the best health and social care outcomes in the country delivered by one of the safest and most efficient systems.
Primary Care Networks (PCNs) will become the vehicle for general practice collaboration with partners who have a stake in improving the health and wellbeing of the local population. They provide the foundation for general practice to build resilience and to operate at the heart of our integrated place-based health and care system which is central to our Locality Plan.
An opportunity has arisen in the Primary Care Transformation team for a 6 month secondment or a fixed term contract. We are looking for an individual, experienced in working with primary care to play a pivotal role in developing and supporting PCNs as the vehicle for general practice sustainability and integrated service delivery in neighbourhoods.
The post holder will support the development and implementation of a number of programmes as identified by PCNs leads and will facilitate opportunities for shared learning, development and planning. The successful candidate will be a highly motivated, enthusiastic individual with excellent communication skills and a proven track record of working with general practice teams.
You will have the opportunity to work as part of a dynamic and forward thinking team that works closely with system partners across Wigan Borough to drive improvements in health and care services.
